As a specialized diagnostic company in hemostasis and sepsis, we would like to share with you views on the affinity of our coagulation tests with the clinical consequences of COVID-19 infection, to emphasize the need to maintain and enhance anticoagulant monitoring of patients as a potential parameter of infection detection-monitoring.
Pneumonia is present in most hospitalized patients with COVID-19 infection and can be developed into sepsis, a known precursor to diffuse intravascular coagulation (DIC). A study by Tang et al, studying changes in coagulation in patients with COVID-19 pneumonia in Yuhan, China, found that 71.4% of those who died and only 0.6% of those who survived showed diffuse intravascular coagulation samples. Based on this result, we can assume that diffuse intravascular coagulation is a common symptom in patients with severe viral pneumonia, which can lead to death. Recognition of the importance of pathological hemostasis during COVID-19 infection prompted the Global Community for Thrombosis and Hemostasis to issue interim guidelines on the detection and management of the associated disorder. In the presence of the patient the d-dimer, prothrombin time, platelet count and fibrinogen should be monitored. The markedly increased D-dimer justifies admission to the hospital even in the absence of other serious symptoms, as it already means increased thrombin production. These hemostasis parameters should be monitored once or twice a day, as worsening of the results indicates the need for a more aggressive therapeutic approach.
Based on the above, it seems that the measurement of hemostasis parameters such as PT, APTT, Fibrinogen and D-DIMER as well as the parameter of Procalcitonin for the sepsis (available from our company), can be checked in the phase of detection-monitoring of infection by COVID-19.
